ComForcare Jacksonville Senior Services Private Duty Homecare
ComForcare Senior Services provides professional personal care and companionship services allowing our clients to safely remain in their own homes, senior living community or medical facility. Clinical research has revealed that people stay healthier and recover more quickly from illness or surgery in the comfort of their own home. However, today's busy lifestyles can deprive family and friends of the time and energy required to keep a loved one safely at home. Fortunately, we provide the necessary help to meet all of the daily needs of the home-bound individual.
The primary Home Health Care Services are: - Personal Care Services - Assistance in the bathroom - Dressing and oral hygiene - Recording vital medical signs - Hands-on Bathing - Transfers (walkers, wheelchairs, beds, showers) - Non-medical and medical transportation - Light Housekeeping - Meal Preparation, grocery shopping and feedings - Medication Reminders - Range of Motion Exercises
All above services are provided by licensed CNA (Certified Nurse Assistants and/or HHA (Certified Home Health Aides).
Companionship Services: - Meal preparation - Light Housekeeping, laundry - Non-Medical and Medical Transportation - Grocery shopping - Assist with in or outdoor activities - Companionship
All above services are provided by carefully screened, experienced companions.
Service Intake Process Steps
Step 1. Free In-Home Assessment Upon the initial phone call, ComForcare's schedulers will make an appointment at the potential client's home with one of our RNs to assess your individual case.
Step 2. Assessment The client's health and ADLs ( activities of daily living) are reviewed to determine the appropriate care plan. If the client's needs require a nursing home, we will make the appropriate referral. In most instances, we proceed to a care plan for future service.
Step 3. Plan of Care and Signing the Agreement A written care plan is developed that outlines specific services appropriate for the new client, complete with specific instructions. The plan is reviewed with legal guardians or other responsible family members and service agreements are signed for immediate or future service.
Step 4. Caregiver Selection and Orientation ComForcare's schedulers will match a caregiver to a client based on skills needed for care, personality and availability. Caregivers are oriented to the client with detailed explanations of special needs and requirements.
Step 5. Monitoring and Review Our Director of Nursing (RN) calls caregivers and clients to monitor care, client condition and provide additional assistance. Regular visits are made to client's homes to also review the care plan and assess the client and caregiver relationship. Caregivers perform daily documentation of client's care and special needs and relay special circumstances to our Director of Nursing for discussion with families and guardians.
