ComForcare Senior Services is a
premier provider of private duty, non-medical home health care in Jacksonville, Florida.
Our specialty is providing excellent care to our eldercare
population. Our caregivers provide personal care for
individuals seeking assistance with ADL's (Activities of
Daily Living). We provide these services in many settings
including residences, assisted living facilities,
independent living facilities, nursing homes, hospitals or
wherever our clients reside. Our goal is to employ and
provide our clients with the highest quality caregivers

established in 1995 as a premier provider
of private duty, non-medical home care with over 70 branches

Highest Standards
ComForcare maintains extremely
high and specific standards regarding the care that we
provide. We are licensed by the State of Florida, Agency for
Health Care Administration (AHCA) with a Home Health Agency
and Home Maker and Companion license.
The Process
The ComForcare Senior Home Care
Admission Service Agreement is completed to begin the
application process. A registered nurse from our agency then
meets with the prospective client/family to write a
comprehensive assessment that outlines the care plan and
specific client needs. A copy of the assessment stays in the
client’s home and serves as a guide for the ComForcare
caregivers. The registered nurse (RN) provides case
management by staying in touch with the client, the
responsible party, and the caregivers. The RN makes regular
home visits to the client to reassess needs and levels of
care. The ComForcare Report, a New Service
We have recently introduced a
new service, ComForcare Report. Our clients’ loved ones
receive monthly updates on the clients’ health and wellness
status prepared by the Director of Nursing. These updates
can be accessed via a special computer web page with privacy
and confidentiality protected,
comforcarereport.com, or they can be received by mail
depending on the family's request. This service is
free and most helpful to long-distant family members of
client's we serve here in Northeast Florida.

Liability and Worker's Compensation
Two levels of service:
Companions
Provide companion services, errands, meal planning and preparation,
non-medical transportation and light housekeeping. They do not
provide “hands-on” care.
Certified Nurse
Assistants/ Home Health Aids
Provide “hands-on” care
including assistance bathing, toileting, and transfers for
the client. They can also provide socialization,
transportation and light housekeeping.
ComForcare can provide as few as four hours daily to as much
as 24 hours of care or live-in care. Costs are dependent upon
the level and duration of the care.
